There are certain items in your licensure packet (which you can pick up from your housestaff office) that will be completed for you by the Student Affairs Office. You will need to mail us the following:
- Certificate of Medical Education (L2 Form) - Place your full name at the top of the form, enter social security number and birthdate. Please note: if you were a transfer student you will have to have your previous medical school complete an L2 form as well for the years that you attended there. Please xerox the L2 form and forward a copy to them for completion with a picture attached as well.
- Transcript - You will need an official copy of your transcript sent to the Medical Board of California along with your application for licensure. The transcript can be sent directly to the Board, and there is a $6.00 charge for an official transcript. Please make the check payable to UC Regents. Please note: if you were a transfer student you will also need to request a transcript from your previous medical school.
- Diploma - Although the Board requests that you send your original diploma to them, they will accept a copy that has been "certified" and "sealed" by our office. Please send a copy of your diploma with your licensure request, and it will be forwarded with the application to the California Board.

The remainder of the licensure application packet should be sent from you. These three items (L2 form, transcript and diploma) will be forwarded directly to the Board.
Please note:
- You are required to submit official transcripts from the undergraduate institution(s) you attended. You must request these directly from your undergraduate institution.
- Forms L5 & L6 can be discarded. Those forms are only to be completed by individuals who graduated from a foreign medical school.
- The best value for your money is to obtain your license during your birth month because then you will receive the full 24 months of licensure. If you can't obtain the license during your birth month, the license will be valid for two birthdays and expires on the last day of your birth month. For example, if your birthday is in November, 2002 and you get licensed in September, 2002 your license will only be good until November of 2003. But if you were licensed in November, 2002 it would be good until November, 2004.
