The DGSOM 2012-13 Financial Aid Application is here! You must be a newly admitted or current DGSOM student to apply. Choose one of two application options:
Option A - Apply for School of Medicine need-based scholarships and loans, in addition to Federal Direct Loans. With this option you're required to provide parent information on the Need Access Application, and other application components.
Option B - Apply only for Federal Direct Loans (Unsubsidized Direct and Grad Plus); with this option you are not required to complete Need Access or provide parent info on any part of the application.
Ready to begin…choose Option A or B and follow the steps below. Each on-line application and form has instructions. Here is an Application Checklist to assist you.
Deadlines only apply to continuing students.
Option A: Apply for SOM scholarships & loans, and Federal Direct Loans:
1. FAFSA / Renewal FAFSA at www.fafsa.gov - Due March 19, 2012
- Click on the link to complete your Renewal FAFSA, which already has last year's data
- Provide student (& spouse's) info; parent info is optional on FAFSA
- Estimate 2011 income on FAFSA if you have not yet filed taxes
- Our FAFSA school code is E00374
- Print & turn in to 12-109 CHS the FAFSA Confirmation
2. Need Access Application at www.needaccess.org - Due March 19, 2012
- Click on the link to complete your Need Access Application
- Provide both student and parent information on Need Access
- Estimate 2011 income if taxes have not yet been filed
- Provide non-custodial parent info on the Divorced/Separated Parent Section
- Print & turn in to 12-109 CHS the Need Access Submission Confirmation
3. Institutional Application - Due March 19, 2012
- Open and print the Institutional Application
- Complete, sign and obtain parents’ signatures
- Turn into the office (12-109 CHS)
4. Tax Certification - Due April 23, 2012
- Open and print Tax Certification
- Complete, sign and obtain parents’ signatures; this form is used to verify tax filing status
- Turn into the office (12-109 CHS), along with copies of signed tax forms (see below)
5. Submit copy of your 2011 Federal Tax Form to 12-109 CHS - Due April 23, 2012
- Turn in a copy of your signed 2011 IRS 1040, 1040a or 1040ez, including all schedules.
- If you are a non filer, submit a copy of your 2011 W2
6. Submit copy of parents’ 2011 Federal Tax Form to 12-109 CHS - Due April 23, 2012
- Turn in a copy of your parents’ signed 2011 IRS 1040, 1040a or 1040ez, including schedules.
- If parents are non filers, print, complete and turn in to 12-109 CHS the Parents’ Statement of Expenses and Resources
- If parents filed an extension, turn in a copy of the Tax Filing Extension Form; you can turn in their 2011 taxes later once they've filed
7. If parents are divorced / separated, the non-custodial parent must: - Due April 23, 2012
- Print, complete, sign and turn in to 12-109 CHS the Non-Custodial Parent Form
- Submit a copy of their signed 2011 IRS 1040, 1040a or 1040ez tax return, including all schedules
- If non filer, print, complete and turn in to 12-109 CHS the Parents’ Statement of Expenses and Resources
Option B : Apply only for Federal Direct Loans - Unsubsidized and Grad PLUS:
1. FAFSA / Renewal FAFSA at www.fafsa.gov Due March 19, 2012
- Click on the FAFSA link and complete your Renewal FAFSA; which already has last year's data
- Provide student (& spouse's) info; estimate 2011 income if you have not yet filed taxes
- Our FAFSA school code is E00374
- Print & turn in to 12-109 CHS the FAFSA Confirmation page
2. Institutional Application - Due March 19, 2012
- Open and print Institutional Application
- Complete, sign and turn into the office (12-109 CHS) - no parent signatures required
Note: Only FAFSA and Institutional Application are required to apply only for Federal Direct Loans
