Forms for VA:

In order to complete a rotation at the Department of Veterans Affairs (VA) Greater Los Angeles (GLA) Healthcare System, all medical students must complete an application packet, which includes:

          • Application for Health Professionals Trainees: APP 102850 D

Please make sure to sign and date page three (questions 26 A & 26 B) and page four.

In addition to the VA application packet, each academic year, ALL medical students must complete the online “VHA Mandatory Training for Trainees released 2010” (first time users/new trainees) or the “Refresher Course for VHA Trainees-2010” (refresher course/returning trainees” as well as the “Authorization for Computer Access (AIS) 2008 V22” Form.  Instructions for completing all necessary computer access documentations are as follows:

  5. Once you are registered you will be automatically routed to the Mandatory Training for Trainees (MTT) course. 
  6. Since the training is only valid for 365 days you should wait until shortly before your first rotation at GLA to complete the MTT or it may expire which would make it necessary for you to redo it.
  7. Trainees that rotate for multiple years will be required to take a refresher course every 365 days.

After an application packet is reviewed and processed by the Office of ACOS/E, medical students will be instructed to contact the VA Human Resources (HR) Department prior to the start of the rotation to schedule a HR and Security processing appointments to have your fingerprints taken, ID checked, and be sworn in.  No trainee will be allowed to start a rotation until all processing requirements are met.

INCREASED PARKING ENFORCEMENT    

To All Employees/Staff:

The VA Greater Los Angeles Healthcare System is in a growing stage which is bringing more patients to all our GLA facilities.  The patients coming for medical care are discovering parking is at a premium in the provided parking lots.  Free access by patients is being further impeded by employees and outsiders who are parking in the designated patient/visitor lots.  This memorandum is a reminder of the existing GLA Corporate Policy No. 07B-03, Motor Vehicle Traffic and Parking Policy.

All employees, visiting medical personnel, vendors and contractor/subcontractors whose vehicles are found parked in designated patient/visitor parking areas are subject to California Vehicle Code 22651(n), which cites the VA Police authority to remove unauthorized parked vehicles, and all costs will be the responsibility of the owner/driver of the motor vehicle.  In addition to the vehicle being removed from the property, the driver will be issued a United States District Court Violation Notice for being in violation of 38 CFR 1.218(b)(29), Parking in spaces posted as reserved or in excess of a posted time limit.  In accordance with the GLA policy and the Master Agreement (AFGE Article 25, Section 4), all employee’s vehicles which are registered with the VA Police Service will receive two (2) warning notices before their vehicle will be towed.

For those unfamiliar with the current policy, you may refer to GLA Policy 07B-03, Motor Vehicle Traffic and Parking Policy.  Within the current policy and in accordance with the Master Agreement, all employees are required to register their vehicle within twenty-four (24) hours after reporting for duty.  Any vehicle which is identified as an employee’s vehicle and is not registered with the VA Police Service is not protected by the AFGE Master Agreement, and will not be afforded two (2) warnings. Those vehicles will be subject to immediate removal/issuance of a U.S. District Court Violation Notice.

To initiate this enforcement, all employees are to be reminded that the VA Police Service is empowered to enforce Federal laws and applicable regulations under the Code of Federal Regulations (CFR) 1.218, concerning the operation and parking of motor vehicles on Government property.

All parking lots designated for patients and visitors are identified by clearly posted signs (parking lots 42, 43 and the Los Angeles Ambulatory Care Center’s lot).  Parking lots designated for employees are provided and also identified by clearly posted signs (parking lots 2, 3, 6, 6A).

Please help in assisting the VA Police by complying with the policies and reporting suspected parking violations.  Any questions can be directed to the VA Police at extension 84517.

Computer Access Instructions (Updated information 12/17/2010)

In order to obtain computer access at the VA Greater Los Angeles Healthcare System, you will need to complete 2 steps BEFORE the first day of your rotation at the VA:

  • Step 1:  Please complete and sign the attached “Authorization for Computer Access (AIS) 2008 V22” form.    
  • Step 2:  Complete the course “VHA Mandatory Training for Trainees Released -2010” for new users (trainees) or the “Refresher Course for VHA Trainees-2010” for returning users (trainees).
    • These courses are available through the VA Employee Education System (EES) website.  Instructions for accessing this website are described below.  After you complete the appropriate course, print out the on-line certificate as proof of completion.

After you have completed steps 1 and 2, please bring all three documents with you on the first day of your rotation.  You will be asked to show both documents in order to receive VA computer access.

    1. First time users must take the “VHA Mandatory Training for Trainees Released – 2010” (the full course).  This full training course includes all 14 prior modules and one short additional module (compliance and business integrity).

 

    1. Returning users must take the “Refresher Course for VHA Trainees- 2010” (the short course).  This refresher course includes only content that must be re-taken each year (to be used ONLY after the long version has been passed once).

 

  • Search for your course by entering one or more keywords such as “VHA Mandatory Training for Trainees Released-2010” or “Refresher Course for VHA Trainees-2010” and click “Search” button.

In order to obtain computer access at the VA Greater Los Angeles (GLA) Healthcare System, you will need to complete 3 steps BEFORE the first day of your rotation at the VA:

  • Step 2:  Please read and sign the last page of the "DVA National Rules of Behavior-VA Handbook 6500 Appd G.doc”.  This document can be downloaded from the UCLA VA Forms website, http://www.medstudent.ucla.edu/offices/sao/clinical/VAForms.cfm
  • Step 3:  Complete the course “VHA Mandatory Training for Trainees – VII”.  This course is available through the VA Employee Education System (EES) website.  Instructions for accessing this website are described below.  After you complete this course, print out the on-line certificate as proof of completion.

After you have completed steps 1 to 3, please bring all three documents with you on the first day of your rotation.  You will be asked to show all 3 documents in order to receive VA computer access.

 

Instructions for Completing Step 3 (VHA Mandatory Training for Trainees course)

  1. Go to the Online Training website https://www.ees-learning.net/librix/loginhtml.asp?v=librix
  2. Click on First Time User.
  3. Create login information.  If you have problems contact the Help Desk at eeslibrixhelp@va.gov or 1-866-496-0463 between the hours of 8:00 am and 10:00 pm EST.
  4. After registering, go back to the Login screen
  5. Enter your username and password, and click on Login. You will be taken to the My Courses page.
  6. Click Available Courses on the left-hand navigation just above My Courses.
  7. Search for course by entering the keywords VHA Mandatory Training for Trainees and click Search button.
  8. You will be taken to the search results page.
  9. Click on the Sign Me Up link on the right side of the screen and you will be taken back to the My Courses screen, where the course will now be listed.
  10. Click on the course name link to go to the course.
  11. Complete the course and print a hardcopy of the certificate to take with you to the VA on your first day.
