Forms and Procedures

Types of Leaves of Absence

  1. ACADEMIC (Student initiated). Examples include, but are not limited to:
    1. Combined Programs, e.g. MSTP, MD/MBA, MD/MPH
    2. Pursuit of another degree, e.g. PhD, JD, etc.
    3. Research, e.g. HHMI
  2. MEDICAL (Initiated by student, student's physician, or Dean. Permission to return to curriculum must also be approved by a physician). Examples include, but are not limited to:
    1. Illness
    2. Maternity
    3. Psychiatric/Cognitive Issues
  3. PERSONAL (Student initiated). Examples include, but are not limited to:
    1. Family Problems, e.g. care-giving or death
    2. Travel
    3. Re-evaluation of Career Goals
    4. Financial Difficulty
  4. ADMINISTRATIVE (Initiated by the Promotions Committee, Faculty Executive Committee, or Senior Associate Dean for Student Affairs, or designee). Examples include, but are not limited to
    1. Honor Code Infractions
    2. Noncompliance with Health Clearance Requirements
    3. Nonpayment of Tuition/Fees
    4. Remediation, e.g. USMLE failure
    5. Required year off for remediation/counseling, etc.

Procedures for Requesting a Leave of Absence / Reinstatement Procedures for Placing Students on Administrative Leave for non-payment of Tuition/Fees

  1. At payment deadline each semester (which is established by the general campus), the medical school registrar requests a list of registered students.
  2. List of students with unpaid balances is reviewed with medical school financial aid office.
  3. Students who have not met their financial obligations to the medical school are contacted by the medical school registrar and given written notification they have five business days to resolve their accounts. This also applies to students whose applications for financial aid applications are pending.
  4. If ten business days pass and account is not resolved, the Senior Associate Dean for Student Affairs is notified and a meeting is scheduled with the student
  5. Students whose tuition/fees remain unpaid three business days after this meeting are removed from courses and placed on Administrative Leave.
  6. A letter is mailed via overnight service to the student outlining the details and reasons behind placing them on Administrative Leave and how they may re-matriculate.
  7. Concerned parties are also notified.
    1. Financial Aid Office
    2. General campus registrar
    3. Other Dean if applicable (MSTP, combined program)
    4. Course instructors
    5. Affected staff members
